Missing a doctor's appointment because you don't have a ride is more common than most people think, especially for older adults and people with disabilities. If you have MassHealth, you may be able to get non-emergency rides to medical appointments through a PT-1. Here's how it works.
What is a PT-1?
A PT-1 is the MassHealth form used to request non-emergency transportation for a MassHealth member to covered medical services. It isn't something you fill out yourself. Your doctor or other medical provider completes it and submits it to MassHealth.
Who qualifies?
In general, PT-1 rides are for MassHealth members who:
- Need to get to a medical appointment or service that MassHealth covers, and
- Don't have another way to get there, such as their own car, a family member who can drive them, or public transportation they're able to use.
Coverage depends on your MassHealth plan and situation. If you're not sure, ask your provider's office or call MassHealth.
How to get a PT-1, step by step
- Ask your provider. At your next visit, or by phone, tell your doctor's office you need transportation to your appointments and ask them to submit a PT-1.
- Your provider submits the request. The office sends the PT-1 to MassHealth with details about your appointments and your medical need for a ride.
- Wait for approval. Once the request is approved, rides can be scheduled for the appointments it covers.
- Book your rides. Give the transportation provider your appointment date, time, and address, as far ahead as you can.
Tips for smooth rides
- For recurring appointments, such as dialysis, therapy, or treatment, ask your provider to include the full schedule.
- Have your MassHealth ID number ready when you call.
- Tell the transportation provider ahead of time if you use a walker or cane or need help getting in and out of the vehicle.
- Plan for the return trip as well as the ride there.
PT-1 is for non-emergency transportation only. In a medical emergency, always call 911. MassHealth rules can change, so check current details with your provider or MassHealth.
