The Role
What You'll Do
- Assess new clients and reassess as needs change
- Create and update individualized care plans
- Provide clinical oversight and guidance to caregivers
- Communicate with families, physicians, and other providers
- Help make sure care is safe, consistent, and well documented
About You
What We Look For
- Active Massachusetts RN license
- Home care, community health, or case management experience is a plus
- Strong assessment, communication, and organization skills
- A reliable way to get to clients' homes
Schedule: Full-time, part-time, and nights/weekends available.
Location: Clients' homes in Lowell, Chelmsford, Tewksbury, Dracut, Tyngsborough, Dunstable, Groton, Pepperell, Billerica, Westford, Methuen, Andover, and Lawrence, MA.
